Appointments: Notify us 24+ hours in advance if you can’t make it. Repeated no-shows, cancellations or lateness may result in dismissal.
Insurance & Payment: Know your plan’s coverage. Bring insurance cards and pay co-pays at the time of visit or provide this information during your online check-in. SOS provides online billing and will send your statement via text or email.
Special Cases: Additional steps apply for Workers' Comp, No-Fault, Medicare Advantage, Disability, or Self-Pay patients. Notify us if this applies to your visit.
Minors: A parent or legal guardian must attend and provide insurance/payment info.
What to Bring to Your Appointment:
- List of all medications (Prescription or not)
- Be prepared to give your medical history
- Your own list of questions to ask your provider

Office Policies: No smoking, weapons, or video/photo allowed. Service animals only.
