Dedicated to ensuring you have the safest and easiest experience with your patient account, SOS utilizes Phreesia for our registration process to help you save time and increase privacy at check-in and beyond. You can securely register and also pay your copay from any smartphone, tablet, or computer from the privacy of your own home.
For New Patients
You will receive an email or a text message prior to your scheduled appointment that will allow you to complete all paperwork through Phreesia at your convenience before you arrive.
For Current Patients
A few days before your appointment, watch for a text message or email from Phreesia, our registration platform. Click the link that is provided and follow the simple process as you’re prompted—no username or password required.
Pre-registering on your device will save you on time and help you keep your information up to date . To help make your next visit easier and faster, simply make sure we have your current cell phone number and/or email address in our system.
How to Self Check-In at SOS using the QR Code
- When you arrive at our office, scan the QR code displayed in our waiting area using your phone.
Most smartphones can scan QR codes using the camera, just open it and point at the code. If not, you can download a free QR scanner app from your app store. - Take a seat in the waiting room and follow the prompts on your screen.
- Enter your name and date of birth, then verify your information.
- If your paperwork is complete, you’ll be checked in, and a member of our clinical team will be with you shortly.
- If additional information is needed, your phone will guide you through the necessary steps.
