Patient Registry from SOS

As part of our mission to deliver exceptional patient care, the Syracuse Orthopedic Specialists joint registry tracks your progress and well-being before and after surgery. Patients undergoing total hip replacement, hip resurfacing, total knee replacement, or primary unicompartmental knee surgery will complete online questionnaires asking about joint condition and activities of daily living as well as general health.

These questionnaires will be emailed to you and need to be completed before surgery and at certain visits after surgery. They take approximately 10 minutes to fill out. It is important to answer each question to the best of your ability. Kindly complete these online surveys when they are sent to you.

Frequently Asked Questions:

Why haven’t I received an email about filling out my questionnaires?
These questionnaires are only being sent for total hip replacement, hip resurfacing, total knee replacement, and primary unicompartmental knee surgery. For all other procedures, we are not currently sending questionnaires.

Another possible reason is that your email address is not on file.

What if I do not have an email address?
If you do not have an email address or do not wish to provide one, you will need to complete these questionnaires on a tablet while you are at the office. However, we do recommend that you complete these questionnaires ahead of time via email in an effort to save time on the date of your appointment.


What if I already completed my questionnaires?
If you already completed your questionnaires, there is nothing else you need to do until you receive your next questionnaire via email.


Who will see the results of my questionnaires?
Only your physician, SOS staff involved in your clinical care, and members of the SOS Research Department will be able to see the responses from your questionnaires.
